Non-Hodgkin Lymphoma (NHL) B-Cell aggressive subtype (except mantle cell) Myeloablative autologous, myeloablative allogeneic or reduced-intensity conditioning (RIC) allogeneic HSCT with curative intent may be considered medically necessary: As salvage for patients not in complete remission (CR) after first line chemotherapy induction; To achieve or consolidate a CR in chemotherapy sensitive first or subsequent relapse; To consolidate a CR in patients with an age-adjusted International Prognostic Index (IPI) that predicts a high or high-intermediate risk of relapse.
